How to Create an HPI Template
HOW TO CREATE A GLOBAL TEMPLATE

Start with a Clean Test Patient:
- Use a test patient that has no data (no notes, no information in Patient Management under Meds, Allergies, Social History, Medical Conditions, or Family History).

Create a New Note:
- Open the test patient and create a new note.

Select the Desired Note Type:
- Make sure you are working in the note type you want to use the templates for. (Global templates are note-type specific.)

Enter Note Details:
- Add as much or as little data to the note as needed.
- Ensure the HPI section is cleared of the age/gender information.
- Address each page of the note with as much or as little information as you prefer.

Create the Template:
- Click the blue TEMPLATE button.
- Name your template.
- Click PREVIEW to review the content.
- Click CREATE to save the template.
HOW TO USE A GLOBAL TEMPLATE
Global templates do not overwrite existing information that has been carried forward from previous notes. If a note already contains data under ROS/PE, the template will only add fields or boxes that are empty.
If you want to apply the entire template, you must either:
- Clear All Fields: Clear all fields in the note before applying the template.
- Clear Applicable Fields: Clear only the specific fields you want to replace with the template data.
Steps to Apply a Global Template:

Open the Note:
- Open the patient’s note where you want to use the template.

Access Templates:
- Click the blue TEMPLATE button in the note.

Select a Template:
- Choose the desired template from your list.
- Note: Templates are specific to the type of note you are working on.

Preview and Apply:
- Click PREVIEW to view the template contents before applying them.
- Once satisfied, click APPLY to apply the template to the note.
HOW TO CREATE AN HPI TEMPLATE:

Access the HPI Template Setup:
- Go to Administration > EMR Setup > HPI Templates.
- It is recommended to use Google Chrome when creating HPI templates for the best experience.
- Note: HPI templates are user-specific. To share your created HPI templates with another user, use the Template Clone Tool in EMR Setup.

Create a New Template:
- Scroll to the bottom of the page and click Create New Template.
- Name your template.

Enter Template Content:
- Free-text the desired content for your template.
- Use only the two features highlighted in the attached screenshot:
3a. "DB Fields" (Yellow):
- Insert demographic data from the patient chart.
3b. "Choice Tags" (Blue/Purple):
- Create predefined choices for the user to select answers to the questions.
- Multi-Select NO (Blue): Only one answer can be selected.
- Multi-Select YES (Purple): Allows selecting multiple answers.
- Add up to 15 choices using the Add One More option.

Save and Add the Template:
- Click Create to save the new template.
- Select Add to add your newly created template to the system.

Editing Existing Templates:
- Follow the same steps above to edit an existing template.

Apply the HPI Template in a Note:
- To apply the HPI template in a note, refer to the instructions below. Ensure you follow Step 5 carefully.
HOW TO USE AN HPI TEMPLATE:
- Open the note and select the desired HPI template from the drop-down menu.
- Choose the relevant tags (if applicable).
- Click SAVE AND EDIT to apply the selected template.
- Once the template is saved, you can enter additional information directly into the HPI template box.
- Click SAVE AND EDIT again to save any changes (IMPORTANT: This step is required even if no additional information was entered).
- (Optional) To use multiple HPI templates, select "Add New HPI Template" from the box in the far-right corner. This will create a second Chief Complaint and HPI box.
HOW TO CLONE TEMPLATES
- Go to ADMINISTRATION > EMR SETUP > TEMPLATE CLONE TOOL.
- Select Clone Type: HPI TEMPLATES.
- Choose CLONE FROM – the user from whom the templates will be copied.
- Choose CLONE TO – the recipient of the templates (staff or specific user).
- Click the CLONE button.
- A confirmation will appear, showing the number of templates cloned.
