Quality Measures Education

Each Education session with the practice will cover 1-2 measures. Similar measures. or measures with similar workflows can be grouped together

For Example: Current Medications & High Risk Medications in the Elderly both pertain to medications, and do not vary widely from regular workflow. Blood Pressure Screening & Controlling High Blood Pressure both involve documenting the patient's blood pressure. Influenza & Pneumonia Vaccinations are both vaccinations and involve the same workflow.

There will be 4 Education sessions, covering all QM's in the span of a month. Attached are the Powerpoints to guide each session. Presenters will also physically navigate around the program to show where entry points are in the course of a visit.

After the Education session, The slides, video and guide will be sent to the practice so that they can review again, or go more in depth with the guide. All Video Documentation links should be present in the powerpoint.
