MDToolbox ID Proofing Troubleshooting: Fraud alert on their credit profile.

The provider had trouble completing ID Proofing. This is due to a fraud alert on their credit profile. They will need to temporarily remove the fraud alert using Experian's website or contact the National Consumer Assistance Center at 855-246-9409.

The following information will be required. They can either mail in the documents to Experian or use the Experian.com/upload website to upload the documents.

Once they remove the fraud alert, they can then try to complete ID Proofing in MDToolbox again.
