Clinical Report Card

The Clinical Domain slide used in the Account Management report is dependent on the provider's MIPS eligibility status. To determine if a provider is eligible as an individual or not:

• Go to https://qpp.cms.gov/participation-lookup?npi
• Enter the provider's National Provider Identifier (NPI) number and click Check All Years.'
• Look up the result under the current Performance Year. A provider is eligible if the result shows looks like this:
